目的 探讨益生菌联合蓝光照射治疗新生儿黄疸的效果。方法 采用便利抽样法选取2023年2月至2024年2月于郑州大学第一附属医院新生儿重症监护室收治的150例黄疸患儿作为研究对象,按治疗方式分为对照组与研究组,每组75例。对照组实施蓝光照射治疗,研究组实施益生菌联合蓝光照射治疗。比较两组临床治疗效果、退黄时间、营养状况(体质量增加量、吸吮次数、白蛋白水平)、肝功能指标[间接胆红素(IBIL)、直接胆红素(DBIL)、总胆红素水平(TBIL)、丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、血清γ-谷氨酰转肽酶(GGT)]水平及不良反应。结果 研究组治疗有效程度优于对照组,差异有统计学意义(P<0.05),两组治疗总有效率比较,差异未见统计学意义(P > 0.05)。相较于治疗前,研究组退黄时间短于对照组,体质量增加量及治疗后吸吮次数均高于对照组,差异有统计学意义(P<0.05)。治疗前及治疗后,两组患儿白蛋白水平比较,差异未见统计学意义(P > 0.05)。相较于治疗前,两组治疗后IBIL、DBIL、TBIL、ALT、AST、GGT水平均降低,且研究组均低于对照组,差异有统计学意义(P<0.05)。研究组不良反应总发生率低于对照组,差异有统计学意义(P<0.05)。结论 益生菌联合蓝光照射在治疗新生儿黄疸临床效果好,不仅能有效改善患者的肝功能,还能提升营养状态,并减少不良反应的发生。
Abstract:Objective To investigate the effect of probiotics combined with blue light irradiation on neonatal jaundice. Methods A total of 150 children with jaundice admitted to the Neonatal Intensive Care Unit of the First Affiliated Hospital of Zhengzhou University from February 2023 to February 2024 were selected as the research subjects by convenience sampling. They were divided into the study group and the control group according to different treatment methods,with 75 cases in each group. The control group received blue light irradiation treatment,while the study group received probiotics combined with blue light irradiation treatment. The clinical therapeutic effects,jaundice regression time,nutritional status( weight gain,sucking frequency,albumin level),liver function indicators [indirect bilirubin( IBIL),direct bilirubin( DBIL),total bilirubin level( TBIL),alanine aminotransferase( ALT),aspartate aminotransferase( AST),serum γ-glutamyl transpeptidase( GGT) ] and the occurrence of adverse reactions of the two groups were compared. Results The effectiveness of treatment in the study group was better than that in the control group,and the difference was significant( P < 0. 05). There was no significant difference in the overall effective rate of treatment between the two groups( P > 0. 05).Compared with before treatment,the time for jaundulation to subside in the study group was shorter than that in the control group,and the increase in body weight and the number of sucking sessions after treatment in the study group were both higher than those in the control group. The differences were significant( P < 0. 05). Before and after treatment,there was no significant difference in albumin levels between the two groups( P > 0. 05). Compared with before treatment,the levels of IBIL,DBIL,TBIL,ALT,AST and GGT in both groups decreased after treatment,and those in the study group were all lower than those in the control group,with significant differences( P < 0. 05). The total incidence of adverse reactions in the study group was lower than that in the control group,and the difference was significant( P < 0. 05). Conclusions The combination of probiotics and blue light irradiation has shown significant clinical effects on neonatal jaundice,which can not only effectively optimize the liver function of patients,but also improve their nutritional status and reduce the occurrence of adverse reactions.
